Effects of concurrent training on muscle strength in older adults with metabolic syndrome: A randomized controlled clinical trial.

Abstract
INTRODUCTION Metabolic syndrome is highly prevalent among older adults. Concurrent training comprises muscle strengthening and aerobic exercise. OBJECTIVE Determine the effects of a concurrent training program on muscle strength, walking function, metabolic profile, cardiovascular risk, use of medications and quality of life among older adults with metabolic syndrome. METHODS A randomised, controlled, blind, clinical trial was conducted in the city of Santos, state of São Paulo, Brazil, involving 41 male and female older adults. The participants were randomly allocated to a control group (n = 18) and intervention group (n = 23) and were submitted to the following evaluations: strength - 1 maximum repetition (1MR) for 12 muscle groups; the Six-Minute Walk Test (6MWT); blood concentrations of cholesterol and glucose; the use of medications; and the administration of the SF-36 questionnaire. The intervention was conducted twice a week over a total of 24 sessions of concurrent training: 50 min of strength exercises (40-70% 1MR) and 40 min of walking exercises (70-85% maximum heart rate). RESULTS Increases in muscle strength were found in the upper and lower limbs in the inter-group analysis and a greater distance travelled on the 6MWT was found in the intervention group (p = 0.001). The intervention group demonstrated a reduction in the consumption of biguanides (p = 0.002). No changes were found regarding metabolic profile, cardiovascular risk or self-perceived quality of life. CONCLUSION The findings of this clinical trial can be used for the prescription of concurrent training for older adults with metabolic syndrome for gains in muscle strength and walking distance as well as a reduction in the use of biguanides.

Vitamin D and calcium supplementation, skeletal muscle strength and serum testosterone in young healthy adult males: Randomized control trial.

Abstract
BACKGROUND Cholecalciferol and/or calcium supplementation might increase skeletal muscle strength and serum testosterone in young adult males. OBJECTIVE We performed a randomized control trial assessing the effect of cholecalciferol/calcium on skeletal muscle strength and serum testosterone in vitamin D deficient young males. DESIGN Two-by-two factorial RCT. SUBJECT AND INTERVENTION Two-hundred and twenty-eight young males were block-randomized to (i) double-placebo, (ii) calcium/placebo, (iii) cholecalciferol/placebo and (iv) cholecalciferol/calcium. Doses for cholecalciferol were 60 000 IU/wk for 8 weeks followed by 60 000 IU/fortnightly, and doses for elemental calcium were 500 mg/twice daily for 6 months. A total of 180 subjects completed the study protocol. Their ean age, body mass index and baseline 25(OH)D were 20.2 ± 2.2 years, 23.0 ± 3.6 kg/m2 and 21.5 ± 9.5 nmol/L, respectively. MEASUREMENTS Handgrip (primary outcome), pinch-grip strength, distance walked in 6 minutes, dyspnoea-score, quality of life by Short Form 36, serum 25(OH)D, 1,25(OH)2 D, iPTH, total testosterone and free androgen index (FAI). RESULTS After intervention, mean serum 25(OH)D was >75.0 nmol/L in cholecalciferol groups. However, the handgrip strength (29.7 ± 4.4, 29.3 ± 4.6, 30.6 ± 5.0 and 28.8 ± 4.3 kg, P = .28) was comparable in the 4 groups. Subgroups analysis among subjects with baseline serum 25OH)D < 25.0 and <12.0 nmol/L showed similar results. The mean serum testosterone decreased significantly at 6 months; however, delta change was similar in 4 groups. Change in handgrip strength and other outcomes was similar in 4 groups with and without adjustment for delta testosterone and FAI. CONCLUSIONS Six months of cholecalciferol/calcium supplementation had no significant effect on skeletal muscle strength and serum testosterone in young adult males.

Abstract
OBJECTIVE High protein (particularly leucine-rich whey protein) intake is recommended to mitigate the adverse effect of weight loss on muscle mass. The effectiveness of this approach is unknown. METHODS Seventy middle-aged (50-65 years old) postmenopausal women with obesity were randomized to (1) weight maintenance (WM), (2) weight loss and the recommended daily allowance for protein (0.8 g/kg/d) (WL group), or (3) weight loss plus whey protein supplementation (total protein: 1.2 g/kg/d) (WL-PS group). Thigh muscle volume and strength were assessed at baseline and after 5% and 10% weight loss in the weight-loss groups and after matched time periods (∼3 and 6 months, respectively) in the WM group. RESULTS A 5% weight loss caused a greater decrease in thigh muscle volume in the WL group than the WL-PS group (4.7% ± 0.7% vs. 2.8% ± 0.8%, respectively; P < 0.05). After 10% weight loss, there was no statistically significant difference in muscle mass loss in the two groups, and the total loss was small in both groups (5.5% ± 0.8% and 4.5% ± 0.7%, respectively). The dietary interventions did not affect muscle strength. CONCLUSIONS Whey protein supplementation during diet-induced weight loss does not have clinically important therapeutic effects on muscle mass or strength in middle-aged postmenopausal women with obesity.

Abstract
BACKGROUND & AIMS In our recent acute metabolic study, we found no differences in the anabolic response to differing patterns of dietary protein intake. To confirm this in a chronic study, we investigated the effects of protein distribution pattern on functional outcomes and protein kinetics in older adults over 8 weeks. METHODS To determine chronic effects of protein intake pattern at 1.1 g protein/kg/day in mixed meals on lean body mass (LBM), functional outcomes, whole body protein kinetics and muscle protein fractional synthesis rate (MPS) over 8-week respective dietary intervention, fourteen older subjects were randomly divided into either EVEN or UNVEN group. The UNEVEN group (n = 7) consumed the majority of dietary protein with dinner (UNEVEN, 15/20/65%; breakfast, lunch, dinner), while the EVEN group (n = 7) consumed dietary protein evenly throughout the day (EVEN: 33/33/33%). RESULTS We found no significant differences in LBM, muscle strength, and other functional outcomes between EVEN and UNEVEN before and after 8-week intervention. Consistent with these functional outcomes, we did not find significant differences in the 20-h integrated whole body protein kinetics [net protein balance (NB), protein synthesis (PS), and breakdown (PB)] above basal states and MPS between EVEN and UNEVEN intake patterns. CONCLUSIONS We conclude that over an 8-week intervention period, the protein intake distribution pattern in mixed meals does not play an important role in determining anabolic response, muscle strength, or functional outcomes. Different Patterns in Muscular Strength and Hypertrophy Adaptations in Untrained Individuals Undergoing Nonperiodized and Periodized Strength Regimens.

J Strength Cond Res 32(5): 1238-1244, 2018-This study investigated the effects of nonperiodized (NP), traditional periodization (TP), and daily undulating periodization (UP) regimens on muscle strength and hypertrophy in untrained individuals. Thirty-three recreationally active males were randomly divided into 4 groups: NP: n = 8; TP: n = 9; UP: n = 8, and control group (C): n = 8. Experimental groups underwent a 12-week strength training program consisting of 2 sessions per week. Muscle strength and quadriceps cross-sectional area (QCSA) were assessed at baseline, 6 weeks (i.e., mid-point) and after 12 weeks. All training groups increased squat 1RM from pre to 6 weeks mid (NP: 17.02%, TP: 7.7%, and UP: 12.9%, p ≤ 0.002) and pre to post 12 weeks (NP: 19.5%, TP: 17.9%, and UP: 20.4%, p ≤ 0.0001). Traditional periodization was the only group that increased squat 1RM from 6 weeks mid to 12-week period (9.4%, p ≤ 0.008). All training groups increased QCSA from pre to 6 weeks mid (NP: 5.1%, TP: 4.6%, and UP: 5.3%, p ≤ 0.0006) and from pre to post 12 weeks (NP: 8.1%, TP: 11.3%, and UP: 8.7%, p ≤ 0.0001). From 6 weeks mid to 12-week period, TP and UP were the only groups that increased QCSA (6.4 and 3.7%, p ≤ 0.02). There were no significant changes for all dependent variables in C group across the time (p ≥ 0.05). In conclusion, our results demonstrated similar training-induced adaptations after 12 weeks of NP and periodized regimens. However, our findings suggest that in the latter half of the study (i.e., after the initial 6 weeks), the periodized regimens elicited greater rates of muscular adaptations compared with NP regimens. Strength coaches and practitioners should be aware that periodized regimens might be advantageous at latter stages of training even for untrained individuals.

The effect of vitamin D2 supplementation on muscle strength in early postmenopausal women: a randomized, double-blind, placebo-controlled trial.

Abstract
BACKGROUND Low serum 25-hydroxyvitamin D [25(OH)D] has been shown to be associated with low muscle mass and loss of muscle strength, resulting in increased disability and frailty in older men and women. Vitamin D deficiency is common in postmenopausal women. The primary objective of the present study was to evaluate the effects of vitamin D supplementation on muscle strength in early postmenopausal women. The effects of vitamin D2 supplementation on muscle mass and muscle cross-sectional area (CSA) were secondarily investigated. METHODS A 12-week, prospective, randomized, double-blind, placebo-controlled trial was conducted in early postmenopausal women (45-60 years old) with vitamin D deficiency (serum 25(OH)D < 20 ng/ml). A total of 88 subjects were randomized into group I: vitamin D2 supplement 40 000 IU/week (n = 44), or group II: placebo (n = 44). Serum 25(OH)D level, muscle strength, muscle mass and muscle CSA were assessed at baseline and 12 weeks after the supplementation. RESULTS After 12 weeks of supplementation, 70% of women in group I achieved a sufficient level of serum 25(OH)D (>30 ng/ml). There were significant differences in changes of serum 25(OH)D levels between the two groups (p < 0.05). Muscle strength and muscle CSA in group I increased significantly after 12 weeks (p = 0.015, 0.045, respectively). However, there were no significant differences in the mean changes of muscle strength, muscle mass and muscle CSA between the two groups (p = 0.16, 0.89, 0.84, respectively). CONCLUSION In this study, we found no obvious effect of vitamin D supplementation on the changes in muscle strength, muscle mass and muscle CSA when compared to placebo. However, there were significant changes in muscle strength and muscle CSA from baseline in the vitamin D supplementation group.

J Strength Cond Res 32(7): 1869-1874, 2018-Whole-body vibration (WBV) and maximum voluntary contractions (MVCs) combined with blood-flow restriction (BFR) to augment postactivation potentiation have yet to be examined. Therefore, the purpose of this investigation was to examine the augmented effects of postactivation potentiation when WBV and MVC are combined with BFR. Twenty men (21.8 ± 2.6 years, 180.5 ± 6.2 cm and 84.5 ± 12.1 kg) completed the study. Participants completed 3 testing sessions in a randomized design that included one of the following: (a) control (CON), (b) WBV and WBV combined with BFR (WBV + BFR), or (c) MVC and MVC combined with BFR (MVC + BFR). Jump height and power were recorded for 3 trials, PRE and POST jump height (cm) mean ± SD for each were as follows: CON 58.9 ± 8.6 and 57.9 ± 8.6, WBV 58.2 ± 8.1 and 59.9 ± 8.1, WBV + BFR 58.7 ± 7.6 and 60.2 ± 8.1, MVC 59.7 ± 7.4 and 60.2 ± 8.6, and MVC + BFR 57.7 ± 7.9 and 59.4 ± 8.1. PRE and POST jump power (W) mean ± SD for each were as follows: CON 1,224.3 ± 221.5 and 1,234.3 ± 189.2, WBV 1,251.1 ± 230.4 and 1,266.1 ± 215.7, WBV + BFR 1,265.8 ± 207.9 and 1,259 ± 223.3, MVC 1,264.7 ± 211.9 and 1,263.5 ± 236.5, and MVC + BFR 1,252.3 ± 222.0 and 1,294.6 ± 256.6. Significant differences were revealed in jump height between the 5 interventions (p < 0.01), WBV (p < 0.01), WBV + BFR (p < 0.01), and MVC + BFR (p < 0.01) revealed significant differences in time but no differences in jump power. In conclusion, the results of this study indicate that WBV, WBV + BFR, and MVC + BFR significantly improve jump height and time in air but not jump power.

High and odd impact exercise training improved physical function and fall risk factors in community-dwelling older men.

Abstract
UNLABELLED High impact exercise programmes can improve bone strength, but little is known about whether this type of training further benefits fracture risk by improving physical function in older people. OBJECTIVES This study investigated the influence of high impact exercise on balance, muscle function and morphology in older men. METHODS Fifty, healthy men (65-80 years) were assigned to a 6-month multidirectional hopping programme (TG) and twenty age and physical activity matched volunteers served as controls (CG). Before and after training, muscle function (hop performance, leg press and plantar- and dorsiflexion strength) and physiological determinants (muscle thickness and architecture) as well as balance control (sway path, one leg stance duration) were measured. Resting gastrocnemius medialis (GM) muscle thickness and architecture were assessed using ultrasonography. RESULTS Significant improvements in hop impulse (+12%), isometric leg-press strength (+4%) and ankle plantarflexion strength (+11%), dorsiflexor strength (+20%) were found in the TG compared to the CG (ANOVA interaction, P⟨0.05) and unilateral stance time improved over time for TG. GM muscle thickness indicated modest hypertrophy (+4%), but muscle architecture was unchanged. CONCLUSION The positive changes in strength and balance after high impact and odd impact training would be expected to improve physical function in older adults.

Effectiveness of water-based Liuzijue exercise on respiratory muscle strength and peripheral skeletal muscle function in patients with COPD.

Abstract
OBJECTS The purpose of this study was to quantitatively assess the effects of water-based Liuzijue exercise on patients with COPD and compare it with land-based Liuzijue exercise. MATERIALS AND METHODS Participants were randomly allocated to one of three groups: the water-based Liuzijue exercise group (WG), the land-based Liuzijue exercise group (LG), and the control group (CG). CG participants accepted no exercise intervention, while training groups performed Liuzijue exercise according to Health Qigong Liuzijue (People's Republic of China) in different environments for 60-min sessions twice a week for 3 months. RESULTS Of the 50 patients enrolled, 45 (90%) completed the 3-month intervention. The CG showed decreased expiratory muscle strength, extensor and flexor endurance ratio (ER) of the elbow joints and flexor peak torque (PT), total work (TW), and ER of the knee joints (p<0.05). Both training groups showed improved respiratory muscle strength, which differed from the CG (p<0.001). In addition, extensor and flexor TW of the elbow joints in the training groups were increased (p<0.01), and the WG differed from the CG in extensor TW and ER and flexor TW (p<0.01), while the LG differed from the CG in flexor TW and extensor ER (p<0.05). PT, PT/body weight (BW), and TW in the knee joint extensor in the training groups were increased as well (PT and PT/BW: p<0.05, TW: p<0.01), and the WG differed from the CG in terms of knee joints outcomes, while the LG differed from the CG in flexor TW only (p<0.05). CONCLUSION Water-based Liuzijue exercise has beneficial effects on COPD patients' respiratory muscle strength and peripheral skeletal muscle function, and additional benefits may exist in endurance of upper limbs and strength and endurance of lower limbs when compared with land-based Liuzijue exercise.

Effects of beta-hydroxy-beta-methylbutyrate supplementation on strength and body composition in trained and competitive athletes: A meta-analysis of randomized controlled trials.

Abstract
OBJECTIVES The aim of this meta-analysis was to examine the evidence for the effectiveness of beta-hydroxy-beta-methylbutyrate supplementation interventions on modification in strength and body composition in trained and competitive athletes. DESIGN Systematic review and meta-analysis. METHODS A systematic search was performed using three databases: MEDLINE, EBSCO and Web of Science. The analysis was restricted to randomized controlled trials that examined the effect of HMB supplementation interventions on modification in bench and leg press strength, body mass, fat-free mass and fat mass. Effect sizes (ES) and 95% confidence intervals (CIs) were calculated using a fixed effect meta-analysis due to low value of the heterogeneity. The Egger test was used to determine the presence of publication bias, and the Q and I2 statistics were used to assess heterogeneity among studies. Significance was set at p<0.05. RESULTS Six studies were selected for meta-analysis, as they fulfilled the inclusion criteria (n=193 participants). HMB supplementation interventions present a trivial non-significant ES in all variables studied (bench press ES=0.00, leg press ES=0.09, body mass ES=-0.01, fat-free mass ES=0.16, and fat mass ES=-0.20; all cases p>0.05, and null heterogeneity I2=0.0% p>0.05). These results remained constant even analyzing by subgroups (HMB doses, duration of intervention, training level and diet co-intervention). CONCLUSIONS This meta-analysis found no effect of HMB supplementation on strength and body composition in trained and competitive athletes.
